Latest News: Monday 08 March
Soon after 12th March 2010, all patients registered with London GPs will be receiving a letter from the NHS informing them that the NHS Summary Care Record (SCR) is being introduced in England. This letter will give details about the SCR and also inform patients that they have the right to opt out and how to do this.
Opt out forms are available at www.london.nhs.uk/noscr or by phoning 0845 603 8510.
Please see the following link to the Connecting for Health website to find more information about the SCR and the implementation programme.

The Summary Care Record website also offers a Frequently Asked Questions (FAQ) page.
The Londonwide Local Medical Committees (LMC) have produced a factsheet for patients and carers called "Your Medical Records - Your Decision". This is also available in the surgery waiting room. It includes an Opt In/Opt Out Form that patients can use to tell us whether they do or do not want to have a summary care record.
Patients have until 21st May to make their choice about opting in or out of the SCR.
We have been informed by NHS Islington that Summary Care Records for this surgery will be introduced over the next 12 to 18 months.
